A grandmother has been charged with attempted murder after she allegedly repeatedly slashed her four-month-old grandson in the face with a meat cleaver, leaving him fighting for his life.

The 64-year-old woman from The Gap in Brisbane's inner-west, will face Brisbane Magistrate's Court on Wednesday.

The boy's mother was visiting a home on Ridgeway Street in The Gap when she woke to the sound of her child crying shortly after 6am on Tuesday.

The four-month-old baby remains in a serious condition at the Lady Cilento Hospital, where he is undergoing surgery for his injuries.

Police photographed a meat cleaver on Tuesday afternoon before submitting it as evidence.

Inspector Daniel Bragg said the boy's mother discovered her son in a 'distressed state' with severe injuries to his face.

The boy's grandmother was found nearby, as was a knife allegedly used in the attack.

'(The mother's) clearly distraught over what's occurred. She's provided police with all the assistance she could,' Inspector Bragg said.

Police and paramedics who rushed to the horrific scene were left 'distressed', said police Inspector Daniel Bragg

Inspector Bragg said both the ambulance and police officers were 'visibly distressed' by the scene

A shocked neighbour said: 'The baby and the daughter arrived about two days ago, they're from down south... (The grandmother)'s very quiet and keeps to herself'

When emergency services were called to the scene, the child was carried into an ambulance before being rushed to hospital.

'This is the worst case scenario. We deal in trauma, we're used to trauma. But probably the most affecting job you can go to is one involving a child,' Inspector Bragg told The Courier Mail

'The ambulance officers who attended were visibly distressed by what they were dealing with... the police officer who was first on the scene was visibly affected by what he saw.'

The attack is being treated as a 'domestic' incident, Queensland Police said.
