FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities during recent inspections from Sept. 7 to Sept. 13.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Elite Academy, 3345 Shannon Park Drive Fredericksburg, VA 22408
    • Date inspected: Sept. 7
    • Total violations: 4
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • A sign or poster that notifies food employees to wash their hands is not provided and/or is not clearly visible at all handwashing sinks used by food employees in the kitchen or the employee restroom.
      • Soap was not provided at the hand washing sink in the kitchen. Soap was provided during inspection.
      • The hand sink and the three compartment sink is not sealed to the adjoining walls.
  • Spotsylvania High School, 6975 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Sept. 7
    • Total violations: 4
      • The Person in Charge was unable to provide documentation of training relating to the Employee Health Policy for this facility Facility provided with Employee Health Policy that was reviewed and signed by all available employees.
      • Observed 8 dented cans in dry storage area of food facility. Dented cans were separated into separate area to be discarded or returned.
      • Restroom door within food facility is not self-closing.
      • Observed several ceiling tiles throughout service area of food facility that are not smooth and easily cleanable.
  • Bob Evans Restaurant, 1020 Stafford Market Pl Stafford, VA 22556
    • Date inspected: Sept. 7
    • Total violations: 11
      • Observed food employee handle clean dishes after handling soiled dishes without washing their hands. Observation was discussed with PIC- dishes were relocated to be washed.
      • Observed raw shell eggs stored in the walk in cooler near ready to eat cooked vegetables. PIC relocated the items.
      • Observed three spatulas and two knives soiled to the touch and sight- items were stored with the clean equipment near the three compartment sink. Items were relocated to be washed, rinsed, and sanitized.
      • Observed the following foods hot holding at improper temperatures at the front of the house: 1) grits (127'F), 2) chicken noodles soup (131'F). Grits was discarded and soup was reheated to 165'F.
      • Observed the following items cold hold at improper temperatures: 1) raw shell eggs (74'F Surface) on the cookline by the grill- the eggs were stored at room temperature for less than 2 hours, PIC placed in WIC, 2) pasteurized liquid eggs stored on the bottom shelf of the prop cooler (47'F)- discarded, 3) cut tomatoes (52'F)- Tomatoes were stored at room temperature for less than 2 hours PIC placed in WIC, 4) a container of sliced boiled eggs on the counter (71'F) discarded. EHS suggested using time as a public health control (TPHC) and provided necessary documents, until the unit is evaluated by a technician.
      • Food establishment is using time for raw shell eggs without prior approval. EHS provided required TPHC document and explained the requirements.
      • Observed several of the cooling units without temperature measuring devices.
      • Observed servers in the front of the house portioning soups, oatmeal, grits, and salads without hair restraints.
      • The prep table by the cookline is in need of repair.
      • The non-food contact surfaces of the interior of all reach-in coolers are noted with an accumulation of grime and debris.

  • JT's Bourbon Street Grille, 5933 B Plank Road Fredericksburg, VA 22407
    • Date inspected: Sept. 7
    • Total violations: 10
      • Observed single-use containers without handles used as scoops in food containers in back prep area of food facility. Containers removed from food items.
      • Observed spray bottle with chemicals stored in bar area of food facility not labeled with the common name of the chemicals. Food employee labeled container.
      • Refrigerators in back prep area of food facility observed without temperature measuring devices available.
      • Observed food container in bar area and in back prep area not labeled with the common name of the food items.
      • Shelving used to dry food equipment in warewash area of food facility observed lined with aluminum foil, not a smooth and easily cleanable material.
      • Sides of fryers observed with an accumulation of debris and residue and are in need of cleaning.
      • Floors and wall around cooking equipment, ceiling and walls in back prep area observed with an accumulation of debris and residue and are in need of cleaning.
      • Flooring in back prep area observed to be damaged and is in need of repair.
      • Ceiling ventilation observed with an accumulation of static dust and is in need of cleaning.
      • Light intensity in warewash area observed at 10 foot candles rather than 20 foot candles as required.
  • Chili's Grill & Bar #1830, 10010 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Sept. 7
    • Total violations: 23
      • The person in charge could not provide verification employees have been trained on the employee health policy. Food employees were knowledgeable of health reporting requirements and information was posted in the office. Provided sample policy to the operator to train staff.
      • The person in charge could not provide written procedures to address clean-up of vomiting and diarrheal incidents. Handouts were provided during routine inspection on 9/8/2021. Sample written procedures were provided to the operator.
      • Observed a food employee don gloves to retrieve baked goods from a reach in cooler without washing their hands. Instructed the food employee to wash their hands and discussed handwashing with the person in charge.
      • Observed ice in the basin of the hand sink by the fryers. Informed the person in charge hand sinks are to be used for handwashing tasks only.
      • Absence of hand drying provision at the hand sink near the fryers. During the inspection the person in charge replaced the disposable paper towel dispenser.
      • Observed numerous foods stored in tabletop steam warmers and the chicken breading station (absence of a lid) without covers to protect from contamination. During the inspection covers were placed on the containers.
      • Observed a food employee open a microwave door which had considerable accumulation of grime and debris wearing a single use disposable glove and then proceed to use the same gloves to handle a spatula at the flat top grill. Instructed the food employee to remove gloves and wash their hands. Discussed correct glove use with the person in charge.
      • Observed the following equipment in contact with Non-TCS foods soiled: Beverage bar gun, equipment to butter rolls, and can opener. The person in charge reported the equipment to butter rolls should be cleaned daily but accumulation of debris on food contact surfaces was observed. The can opener was removed for cleaning, discussed deep cleaning.
      • Through discussion with the person in charge the cutting boards on prep tables, which are used in contact with TCS foods, are removed for cleaning once daily. The person in charge reported the cutting boards are sanitized throughout the day. Informed the person in charge equipment in contact with TCS foods are required to be cleaned and sanitized at least once every 4 hours.
      • Observed the interior of multiple microwave ovens and 6 fryer baskets stored above the fryers soiled. The person in charge reported the fryer baskets were from the evening prior. Also observed encrusted debris on pans used in contact with food. Discussed deep cleaning of the pans to remove debris for food contact.
      • Observed commercially prepared marinara sauce (118 degrees F) improperly reheated for hot holding. A food employee reported the marinara sauce was reheated and placed in the warmer at approximately 11:30 a.m. The sauce was removed to the microwave for reheating. Observed reheating temperature of 173 degrees F by 12:00 p.m. The person in charge could not provide temperature logs to verify the reheating temperature.
      • Observed the following TCS foods cold holding at improper temperatures: 1- Prep cooler #2- Pasta (53 degrees F) (top of unit), Pasta (43 degrees F), precooked chicken (52, 48 degrees F), cooked pork ribs (48 degrees F), chicken quesadilla (44 degrees F) , cooked peppers and onions (43 degrees F) and pasta (46 degrees F) in the bottom of the unit. The person in charge reported the TCS foods were pulled from the walk in cooler at 10:00 a.m. and the foods were relocated to the walk in cooler to cool to 41 degrees F or less by 2:00 p.m. The pasta in the top of the unit was found overfilled in the container and was discussed with the person in charge. 2- Lettuce cooler - Observed cut lettuce (45 degrees F), observed the lid to the cooler opened. The person in charge reported the lettuce was placed in the unit at 11:30 a.m. Instructed the person in charge to sell, cool to 41 degrees F or discard lettuce by 3:30 p.m. 3- Observed single service containers of half and half (50 degrees F) cold holding in a pan of ice in contact with the bottom only. The person in charge discarded the half and half containers. Contact technician to evaluate the cold holding capacity of the units.
      • Absence of consumer advisory for old time hamburgers with cheese. The person in charge reported this menu item may be ordered raw and/or undercooked per a customer's request. Update menu to include an asterisk to this item, a reminder is on the menu for other raw animal foods which may be ordered raw and/or undercooked.
      • Observed stainless steel bowls and several stacks of plastic food containers stacked while wet after chemical sanitizing.
      • Observed scratched and scored cutting boards on prep tables which are used in contact with food. The food contact surface is no longer smooth and easily cleanable.
      • Observed the right side temperature gauge on the dishwasher is not working during warewashing operations. The label is eroded on the temperature gauges to read if if was the wash or final rinse temperatures. Contact technician for service.
      • Observed interior components of the dishwasher soiled.
      • Absence of data plate on the low temperature dishwasher.
      • Significant accumulation of grime and debris on all nonfood contact equipment including interior and exterior of cooking and refrigeration equipment, dishwasher curtains, prewash spray nozzles and upsplash of the coffee brewer. Strongly recommend detailed cleaning.
      • The floors, walls, ceilings and floor drains throughout the facility are in need of cleaning. Recommend detailed cleaning in the food establishment.
      • Observed the following in poor repair: Numerous blown out ceiling lights, regrouting of floor tile is needed, ceiling tiles are peeling in the men's toilet room and kitchen, ceiling tiles above the 3 vat sink are not secured to the frame.
      • Observed 13.7 foot candles of light by the cook line, observed multiple blown out ceiling lights.
      • Observed residue on the vent filters above the fryers and flat top grill.
  • Lewis & Clark Elementary School, 18101 Clark & York Boulevard Ruther Glen, VA 22546

      • Observed the following equipment that is in need of repair or replacement: 1-the ice machine is leaking on the floor which has caused the legs to become rusty, preventing proper cleaning, 2-the interior of one of the reach in coolers on the serving line is leaking causing a water build up on the interior, 3-the soap dispenser for the hand sink near the 3 compartment sink is missing (the person in charge said that it was broken and would not stay hanging on the wall).
      • The 3 compartment sink is not sealed to adjoining wall.

    • Date inspected: Sept. 8
    • Total violations: 4
      • Observed boxes of food stored on the floor in the walk-in freezer. Person in Charge put items on shelving.
      • Observed boxes of single-use lids and straws stored on floor in storage room. Observed single-use trays at service line not covered or inverted. Trays were turned over to prevent contamination.
      • Observed water pooling on floor in walk-in refrigerator.
      • Heat lamps above steam tables at service line observed with an accumulation of debris and residue and are in need of cleaning.
  • Riverbend High School, 12301 Spotswood Furnace Road Fredericksburg, VA 22407

    • Total violations: 5
      • Observed boxes of food stored on the floor in the walk-in freezer.
      • Observed ice scoop handle stored in ice at ice machine. Ice scoop was removed from equipment.
      • Observed plastic wrapped single-use trays stored on floor in side storage area of food facility.
      • Observed several ceiling tiles throughout food facility that are not made of a smooth and easily cleanable surface.
      • Observed several ceiling tiles in kitchen area of food facility that have water damage and are in need of replacement.
  • The Pig Pitt, 1017 Sophia Street Fredericksburg, VA 22401
    • Date inspected: Sept. 8
    • Total violations: 6
      • Observed utensils at the hot line being stored in water (62 degrees F) and scooping utensils at the ice cream bar stored in water (55 degrees F). Person in charge relocated utensils and will keep them in the hot food containers and ice cream scoops inside of the refrigerated unit.
      • Observed rolls of paper towels stored out of the box they were received in and not protected from contamination.
      • Observed the racks holding clean equipment in the kitchen require cleaning.
      • Observed the walls behind the 3 compartment sink of the kitchen and the ceiling of the walk in cooler have black growth and require cleaning.
      • Observed the floors of the kitchen are peeling with paint, not smooth and easy to clean.
      • Observed 5 lights out in the back kitchen area and 1 light out at the hood vent in the front kitchen area.
  • Paragon Village 12, 51 Towne Centre Blvd Fredericksburg, VA 22407
    • Date inspected: Sept. 8
    • Total violations: 2
      • Observed standing water on the bottom shelf of the prep unit.
      • The floors in the concession stands were observed soiled. Observed standing water under the beverage dispenser/counter and food debris under the prep table.
  • Burger King #1213 (Plank Road), 3052 Plank Road Fredericksburg, VA 22401
    • Date inspected: Sept. 8
    • Total violations: 7
      • Observed a food employee touch their headset and tshirt with a gloved hand. Employee continued to work without washing hands and changing gloves. Person in charge had a discussion with employee and instructed them to remove their gloves and wash hands.
      • Observed the inside surfaces of the cookie/bacon oven require cleaning.
      • Observed two pans of sliced tomatoes cold holding at improper temperatures (44-52 degrees F) due to improper ambient cooling. Tomatoes were cut at 10 am, time of inspection was passed the 4 hour ambient cooling time frame (inspection time 3pm). During the inspection person in charge discarded the pans of tomatoes for safety.
      • The following non-food contact surfaces were observed HEAVILY soiled: 1. Metal racks holding clean equipment above 3 compartment sink 2. Inside and outside of under counter cooling and freezing units. 3. Outside surfaces and inside cabinets of deep fryers. 4. Outside surfaces and sides of prep stations 5. Wheels/Feet of all equipment in kitchen Overall recommend a DEEP AND THOROUGH cleaning of all non-food contact surfaces of the kitchen.
      • Observed unused and broken down equipment (milk shake machine, chicken breading/batter station) in the facility that should be removed from the premises.
      • Observed a ceiling panel missing in the drive thru area and above the hand sink near the ice machine.
      • Observed the following heavily soiled: 1. Floors underneath and in between all equipment in the kitchen 2. Floors underneath dry storage racks 3. Floors and door threshold of the walk in cooler 4. Floors surrounding mop sink 5. Mop sink basin heavily soiled 6. Walls of the entire kitchen 7. All AC/ceiling vents of the facility, including men and womens bathroom and vents of the ice machine. 8. All floor drains 9. Observed black growth/staining on the ceiling of the walk in cooler. Overall health department recommends a VERY DEEP AND THOROUGH cleaning of the kitchen, to include back storage room, area around ice machine.
  • Bowlero Fredericksburg, 12100 Kilarney Drive Fredericksburg, VA 22407
    • Date inspected: Sept. 8
    • Total violations: 2
      • Chili and cheese dispensers observed with encrusted residue and are in need of cleaning. Person in Charge cleaned equipment food-contact surfaces.

      • Observed food employee wash their hands at the 3 compartment sink. EHS informed food employee of where to wash hands. Food employee rewashed hands at the handwashing sink. (Corrected)
      • Observed an employee wash hands in less than 10 seconds and under running water. EHS informed employee of the proper handwashing procedure. During the inspection, the employee rewashed their hands. (Corrected)
      • Observed a several spoiled red onions and 1 yellow onion that were covered in flying insects. During the inspection, the food employee discarded the spoiled onions. (Corrected)
      • Observed the dishwasher being used without sanitizer. When EHS asked food employee, he stated they were waiting for the sanitizer to arrive for the dishwasher. EHS recommended they wash in dishwasher and sanitizer at the 3 compartment sink. During the inspection, the chlorine sanitizer was set up at the 3 compartment sink. (Corrected)
      • Observed 2 spray bottles of degreaser hanging at the 3 compartment sink that were not labeled. During the inspection, an employee labeled the spray bottles. (Corrected)
      • Observed two dented 6 lb cans of pasta sauce being stored with other non-dented cans on the dry storage rack. During the inspection, the food employee relocated the dented cans. (Corrected)
      • The following are in need of repair: 1. Light out in front of the walk-in cooler. 2. The door handles are cracked/missing on the 2 door reach in cooler. 3. Light out between the reach in freezer and door into the dining area. 4. Standing water on the bottom shelf of the sandwich prep unit. 5. Light out between the dry storage rack and exterior back door.
      • The following non-food contact surfaces are in need of cleaning: 1. The belt on the top conveyor oven. 2. The exterior surface of the dishwasher.
      • The following are in need of repair: 1. Cracked and missing wall tile at mop sink. 2. Several chipped floor tiles throughout the kitchen.
      • The following are in need of cleaning: 1. The light shield over the back exterior door. 2. The walls at the mop sink. 3. The ceiling and wall between the walk-in cooler and pizza prep table. 4. The ceiling filter over the self serve area. During the inspection, the employee cleaned filter.

    • Date inspected: Sept. 9
    • Total violations: 2
      • Absence of portable handwashing station. The owner instructed an employee to go to the store to purchase adequate handwashing supplies.
      • Absence of ground cover under the canopy. The facility is set-up on a grass ground covering. Discussed purchasing a tarp for future events.
  • Sparky's BBQ (VA SPARKY S), 14523 Kings Hwy King George, VA 22485
    • Date inspected: Sept. 10
    • Total violations: 5
      • The facility does not have an employee in a management or supervisor position who is a certified food manager.
      • The person in charge could not provide verification of employee health policy training. Employees were knowledgeable of health reporting requirements. Sample policy will be emailed.
      • The person in charge could not provide written procedures to address clean up of vomiting and diarrheal accidents. Sample written procedures will be emailed.
      • Observed baked beans in the smoker uncovered and contaminated with wood debris from the smoker. The person in charge voluntarily discarded the baked beans. Discussed covering of beans in the smoker during cooking/reheating.
      • Observed 2 spray bottles which the person in charge reported held Chlorine sanitizer which were not labeled. The bottles were labeled during the inspection.
  • Kaziville (KAZIVL1), 7 Kelvin Drive Stafford, VA 22554
    • Date inspected: Sept. 10
    • Total violations: 7
      • Through discussion with the person in charge the chili in the mobile unit was opened from canned goods and stored in a private home and equipment is washed and sanitized in a private home. Observed improper ambient cooling of chili and food temperatures of TCS foods are not being monitored during cold holding.
      • The person in charge could not provided verification of employee health training. Employees were knowledgeable of health reporting requirements
      • The person in charge could not provide a written policy on clean up procedures for vomiting and diarrheal events. Sample policy was provided during routine inspection on April 4, 2022.
      • Observed an employee don disposable gloves to prepare a customers order without washing her hands. Prior to donning gloves she was handling her cellphone. Instructed the employee to wash her their hands.
      • Observed inadequate ambient cooling of chili which was removed from canned goods the day prior. Observed chili at 54 degrees F in the Magic Chef refrigeration. The person in charge reported she would take it home.
      • bserved 2 packages of precooked sausage (54, 54 degrees F) cold holding overnight at improper temperatures on the interior door of the Magic Chef refrigerator. The person in charge reported the food would be taken home.
      • Observed insect repellant stored next to air fryer. The insect repellant was relocated to the cab of the mobile unit.
  • Kobe Japanese Steakhouse and Cafe, 10119 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Sept. 12
    • Additional comment, as written by the health department: "Due to the aggressive and hostility of the owner, communication became difficult and EHS left the premises without being able to review all items requiring review. EHS will attempt to follow up with the remaining corrective actions via email communication."
    • Total violations: 4
      • Observed the following TCS foods cold holding at improper temperatures inside the sushi reach in cooler: Roe (44, 46, 52 degrees F), shrimp tempura (46, 46 degrees F), raw tuna (47 degrees F), cooked octopus (45, 48 degrees F), imitation crab meat (46 degrees F), white tuna (47 degrees F), and smoked salmon (46 degrees F). The person in charge reported the TCS foods were held in the unit overnight and voluntarily discarded for safety. During the inspection a technician was contacted for service. Discussed placing small portions on ice, monitoring internal temperatures and relocating TCS foods to the walk in cooler prior to closing.
      • The menu does not include an adequate consumer advisory for steaks, which may be cooked to order, and raw and/or raw marinated fish used to prepare sushi. The sushi menu has the reminder but not the disclosure. EHS left the establishment before the menu could be reviewed.
      • Food employees could not provide a thin probed food thermometer for monitoring internal temperatures of thin massed foods. The facility has a bimetallic stem thermometer with the temperature sensor halfway up the shaft. EHS left the facility before receipt of a food thermometer could be verified.
      • The person in charge could not provide Quaternary Ammonium testing kit. Observed the facility has Quaternary Ammonium sanitizing solutions. EHS left the facility before verification of a testing kit could be reviewed.
  • Hard Times Café, 5099D Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Sept. 12
    • Total violations: 11
      • During discussion with the person in charge regarding the chicken breading station, they stated that the chicken breading is not sifted, and thrown out after approximately 6 hours on the line. Explained to the person in charge that due to the breading being in contact with raw chicken, the breading needed to be sifted or discarded every 4 hours, or kept at 41'F or below.
      • Observed three spray bottles of unidentified chemicals hanging above the clean side of the dish machine staging tables. Bottles were removed during inspection.
      • Observed several spray bottles of chemicals throughout the kitchen that were not labeled. Bottles were labeled during inspection.
      • Employees observed working in the prep area without proper hair restraints. Employee retrieved a hat from their car during inspection.
      • Clean stainless steel storage pans on the clean equipment rack were not observed stored in a position to allow air-drying.
      • Observed two wire shelving units in the kitchen holding clean equipment that are covered in rust.
      • An irreversible registering temperature indicator was not provided for the hot water mechanical warewashing operation.
      • The following non-food contact surfaces were observed soiled with grime and debris: 1-the condenser box in the walk in cooler, 2-the bulk storage bins, 3-inner surface to the door to the walk in cooler.
      • The ceiling in the dry storage room and several ceiling tiles in the kitchen were observed damaged.
      • The ceiling tiles around the air ducts in the kitchen and the ceiling tiles around the ducts were observed soiled with dust and dirt.
      • Less than 50 foot candles of light was noted at the cook's line. Observed only 10 foot candles in this area. With the line overhead lights on, it only measured 25 foot candles.
  • Hugh Mercer School, 2100 Cowan Blvd Fredericksburg, VA 22401
    • Date inspected: Sept. 12
    • Total violations: 6
      • Observed no paper towels were available at the handsink by the warewashing area. During the inspection person in charge provided paper towels.
      • Observed a food employe attempt to sanitize a washed and rinsed knife at the 3 compartment sink by submerging the knife in the sanitizing solution for less than 2 seconds. During the inspection knife was re-submerged in the sanitizer for the 30 second sanitizing contact.
      • Observed several rolls of hand paper towels stored on a shelf in the dry storage room.
      • Observed several cutting boards heavily stained with black staining.
      • Observed the following are dusty and require cleaning: 1. AC/celing vents 2. Light shields and water suppression lines under hood vent 3. Vents of the back of equipment in cooks line (ovens)
      • Observed less than 1 foot candles in the walk in freezer and in the walk in cooler 3.7 foot candles.

    • Date inspected: Sept. 12
    • Total violations: 3
      • Observed rodent-like feces in mop room of food facility. Facility does have a pest control program.
      • Walk-in freezer door observed not to be closing properly.
      • Mop sink observed without backflow prevention on spray hose. Hose was disconnected until a backflow device can be installed.
  • Taj Indian Cuisine, 2032 Plank Rd Fredericksburg, VA 22401
    • Date inspected: Sept. 13
    • Total violations: 14
      • Person in charge was unable to provide a verifiable way to show food employees have been informed and trained on foodborne illnesses and symptoms associated with foodborne illnesses. During the inspection EHS provided a copy of an employee health policy. During the inspection person in charge trained and had employees sign the employee health policy.
      • Observed no hand soap was available at the bar hand sink. During the inspection soap was provided.
      • Observed no hot water was available at the hand sink of the kitchen. During the inspection the person in charge turned on the hot water valve underneath the hand sink. Discussed having the hot water accessible and available at all times.
      • Observed no paper towels were available at the kitchen hand sink. During the inspection the person in charge provided paper towels.
      • Observed a container with some molded strawberries in the walk in cooler. During the inspection strawberries were discarded.
      • Observed one dented can of ketchup and one heavily dented and punctured at the opening rim can of coconut milk. Mold like substance was observed oozing out of coconut milk can. During the inspection the cans were discarded.
      • Observed several pans and containers of food in the walk in freezer stored uncovered.
      • Observed the soda gun nozzle of the bar area requires cleaning.
      • Observed the following hot holding at improper temperatures: 1.one pot of white rice hot holding at 120-130 degrees F. 2. one pan of garlic oil at the cook line 104 degrees F. Food had been made less than half hour before inspection. Person in charge will keep foods for 4 hours and will discard and will adopt time as a public health control in the future for these two food items. EHS provided time as a public healt control paperwork.
      • The following foods were found cold holding out of temperatures due to a cooling unit in disrepair: 1. one small container of yogurt red sauce-47 degrees F. 2. one small container of shredded cheese-51 degrees F. 3. one small container of feta cheese-47 degrees F. 4. two meat samosas- 47 degrees F. 5. one pan veggie fritters-51-55 degrees F. 6. one container of potato and meat- 56 degrees F. 7. one bag of potato patties-53 degrees F. 8. one bowl of spiced cream cheese -51 degrees F. 9. one bowl of restaurant made mayo- 54-55 degrees F. 10. one container of chicken chunks- 44 degrees F. 11. one container of chicken bites- 43 degrees F. All foods were discarded for safety. With the exception of the chicken chunks and chicken bites. Foods were put in unit less than 1 hour before inspection and were moved to the walk in cooler.
      • Observed one container with various medications stored on a prep table in the kitchen. During the inspection person in charge relocated the container of medications.
      • Observed the metal shelves holding clean equipment in the dishroom area require cleaning.
      • Observed the following in need of cleaning: 1. Walls of the warewashing area 2. Walls and floors of the dry storage area 3. Wall behind the hand sink of the kitchen.
      • Observed the ceiling tiles of the men and womens bathroom is porous and not smooth and easy to clean.
  • Chancellor High School, 6320 Harrison Road Fredericksburg, VA 22407
    • Date inspected: Sept. 13
    • Total violations: 4
      • Observed cookies stored uncovered at service lines throughout food facility. Cookies were covered to prevent contamination.
      • Observed single-use trays at service lines throughout facility that were not stored covered or inverted.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Observed several ceiling tiles throughout food facility that were not smooth or easily cleanable and several were observed with water damage.
  • Edward E. Drew Middle School, 501 Cambridge Street Fredericksburg, VA 22405
    • Date inspected: Sept. 13
    • Total violations: 2
      • Observed the Horbart reach in refrigeration on line 2 not working properly. Observed a food employee placing salads in the unit. Salads were relocated. Instructed PIC not to place any time temperature control for safety foods in the unit until it gets evaluated by a technician. PIC contacted technician during inspection, technician arrived while EHS is typing the report.
      • The concentration of the quaternary ammonium sanitizer solution at the 3 compartment sink and the sanitizer buckets measured less than 100 PPM, according to the manufacture instructions the concentration should be between 150-400 PPM. The person in charge relocated the bottle and stated that an employee might have moved it while cleaning (corrected 300PPM).
  • Brock Road Elementary School, 10207 Brock Road Spotsylvania, VA 22553
    • Date inspected: Sept. 13
    • Total violations: 2
      • Shelving in walk-in refrigerator observed with an accumulation of residue and are in need of cleaning.
      • Ceiling throughout facility is not made of a smooth and easily cleanable surface.
  • James Monroe High School, 2300 Washington Avenue Fredericksburg, VA 22401
    • Date inspected: Sept. 13
    • Total violations: 5
      • Observed several rolls of paper towels stored on a shelf unprotected.
      • Observed a spice rack cart in the kitchen was soiled. During inspection food employees cleaned the spice cart.
      • Observed the dish washing machine is constantly draining, shut off mechanism seems to be in disrepair. Drainage of water is pouring faster than water is allowed to drain. A heavy accumulation of water around drain was observed.
      • Observed the following in need of cleaning: 1. ceiling vents in the dry storage room and in the dish room were heavily dusted. 2. walls behind the dish machine and caulking of the dishroom area.
      • Observed 2 ceiling tiles missing in the dry storage room.
  • Spotsylvania Regional Medical Center, 4600 Spotsylvania Pkwy Fredericksburg, VA 22408
    • Date inspected: Sept. 13
    • Total violations: 2
      • Observed tomatoes stored in prep unit being overstocked and holding at 47F, rather than 41F or below. Tomatoes were discarded.
      • Observed reduced oxygen packaged (ROP) salmon stored in prep unit thawing without having been removed from the ROP environment. Person in Charge discarded salmon.
